To prepare for your visit with The Centers for Advanced Orthopaedics/Maryland Orthopedic Specialists Division please select the demographic document for all in office visits. If you will have a telehealth visit you will need to complete the telehealth authorization form as well as the demographic. Please complete all the documentation and return it to our office with a copy of your insurance card and photo ID prior to arriving for your appointment. Please send the documents to mosfrontdesk@cfaortho.com. Alternatively, you may bring the completed paperwork to the office for an in person visit. You will complete your health history information on our patient portal at cfaortho.ema.md 

If you cannot access the portal please contact our office for access to be turned on. The portal is new as of 1.25.2021.

Telehealth appointments will not happen until the paperwork has been received by our front office staff. This documentation would need to be emailed to mosfrontdesk@cfaortho.com or it can be faxed to 240-912-2382. We apologize for any inconvenience, but this paperwork is required to complete the office visit.

Due to the ongoing COVID-19 Pandemic we ask that you always wear a mask in our office for your safety, as well as, the safety of our staff. Please communicate with our office staff regarding any illness, exposures, or travel as it may be necessary to reschedule your appointment and/or convert your appointment to a telehealth visit. Additionally, we are limiting the number of visitors in our office and request that patients attend office visits alone unless they are a minor, translator, or assisting someone for safety purposes.
