What is animal and human bites to the hand?
Bite wounds to the hand, from dogs, cats, or humans, carry a high infection risk because teeth inject bacteria deep beneath thin skin near tendons and joints. A fight bite from a clenched fist striking teeth is especially dangerous. Prompt irrigation, antibiotics, and sometimes surgery prevent serious infection; delayed care raises the risk substantially.
A bite wound to the hand is a puncture, laceration, or crush injury from the teeth of an animal or a human that introduces bacteria from the mouth directly into deeper structures: skin, fat, tendon, tendon sheath, joint capsule, and sometimes bone. The hand is a uniquely bad place to be bitten. Its tendons, joint capsules, and bone lie close beneath thin skin, especially over the knuckles, so even a wound that looks small on the surface can seed bacteria deep enough to cause a serious infection (JAAOS review of mammalian bite injuries).
One specific injury deserves its own explanation because it is easy to miss: the "fight bite," also called a clenched-fist injury. It happens when a person punches another person in the mouth, and a tooth cuts the skin over a knuckle while the fist is clenched. The danger is in what happens next. Because the wound is made while the joint is bent, the extensor tendon, joint capsule, and cartilage underneath are cut in that flexed position. When the hand is later straightened out, those injured structures slide backward, away from the small skin wound, sealing bacteria deep inside the joint while the surface wound looks deceptively minor. In a review of 191 clenched-fist injuries, 67 percent had direct violation of the knuckle joint (JAAOS 2015), and a separate report found more than half of fight bites penetrate the joint ("Packing a Punch," PMC). Alcohol is frequently involved, present in up to 86 percent of fight-bite cases in one series, which is part of why patients often understate or misremember exactly how the injury happened (PMC9701090). Because of this, we treat any wound over the knuckle sustained during an altercation as a probable fight bite until proven otherwise, regardless of what we're initially told about how it happened.
Symptoms — do you recognize these?
Bite wounds range from a simple puncture with redness around it to a deep infection that threatens the function of the hand.
- A puncture, laceration, or crush wound, sometimes small, from teeth
- Redness, warmth, and swelling around the wound
- Increasing pain, especially in the day or two after the bite
- Pus or other drainage from the wound
- A finger that is uniformly swollen along its whole length, held bent, and painful to straighten (a sign the flexor tendon sheath is involved)
- Fever or feeling generally unwell with a more advanced infection
Any bite to the hand should be seen the same day it happens, not after it starts to look infected. If you already have spreading redness, fever, a finger that is swollen its entire length, or pain when the finger is passively straightened, go to an emergency room or call our office right away rather than waiting for an appointment.
How we diagnose it
Diagnosis is based on a careful history, including what kind of animal or person caused the bite, how long ago it happened, and your tetanus and vaccination status, combined with a hands-on exam of wound depth, whether a joint or tendon sheath was entered, and a check of circulation and nerve function in the finger. We specifically probe any wound over a knuckle sustained in a fight for joint penetration, since patients frequently minimize or misstate how the injury occurred (JAAOS 2015).
X-rays are obtained to look for a retained tooth fragment, a fracture, or, in a wound that has been present for a while, early bone infection. If we're worried about a joint infection, we may aspirate fluid from the joint to send for testing, and wound or blood cultures help direct antibiotic choice once an infection is established (JAAOS 2015).
Treatment options
Treatment depends on the type of bite, how deep it is, and how long ago it happened. The single biggest factor in outcome is how quickly the wound is cleaned and treated.
Irrigation and wound care
Thorough washing out of the wound, typically with a syringe generating enough pressure to flush contamination from the tissue, along with tetanus booster if it has been more than 5 years since the last one.
When used: Every bite wound, regardless of how minor it looks, as the first step in treatment.
Oral antibiotics (amoxicillin-clavulanate)
Amoxicillin-clavulanate is the preferred first-line oral antibiotic for dog, cat, and human bites because it covers the most common bacteria from all three sources, including Pasteurella, Eikenella, staph, strep, and anaerobes, in one medication. A pooled analysis of eight randomized trials found antibiotic prophylaxis reduced hand-bite infection rates from 28 percent down to 2 percent (JAAOS review of pooled RCT data). Duration is tailored to depth: about 10 to 14 days for simple cellulitis, roughly 3 weeks for tendon sheath infection, about 4 weeks for joint infection, and roughly 6 weeks for bone infection (JAAOS 2015).
When used: Nearly every hand bite, started as early as possible; a StatPearls review notes antibiotics given within 6 hours of the bite cut infection risk to roughly 8 percent, versus about 59 percent when treatment is delayed (StatPearls, Animal Bites).
Rabies risk assessment
A discussion of the biting animal's species, vaccination status, and behavior, together with local public health guidance, to decide whether rabies post-exposure treatment is needed. Dogs are the leading global cause of human rabies, but in the United States the main reservoirs are bats, raccoons, skunks, and foxes (JAAOS 2015).
When used: Any bite from an animal whose vaccination status is unknown or that behaved abnormally.
Delayed or no primary closure
Leaving the wound open, or closing it loosely after a period of observation, rather than stitching it tightly closed right away. Primary closure under tension traps bacteria in a closed space.
When used: Most hand bites, deep puncture wounds (especially cat bites), wounds seen more than 24 hours after injury, and any wound already showing signs of infection should generally not be closed tightly at the first visit (JAAOS 2015).
Recovery & rehabilitation
How quickly a bite is recognized and treated has more influence on the outcome than almost anything else. In one series, early washout of cat bites reduced the infection rate to as low as 0.5 percent, compared with 37.5 percent when treatment was delayed (Open Orthopaedics Journal, PMC). A retrospective review of 124 human hand-bite patients likewise found that outcomes tracked closely with how much time passed between the injury and treatment (JAAOS 2015).
Most simple bites treated promptly with irrigation and antibiotics heal without lasting problems over 1 to 2 weeks. Bites requiring surgery, such as a fight bite with joint involvement, generally need weeks of antibiotics plus hand therapy to restore full motion once the acute infection has cleared. Delayed presentation, joint penetration, and a weakened immune system are the main risk factors for a worse outcome, including the need for repeat surgery or, in severe untreated cases, lasting stiffness or loss of function (JAAOS 2015).
